Insurance
Information You need to be aware that your policy is a contract. If you do not have a copy of your policy in its entirety, PLEASE obtain one as soon as possible. We recommend that you read your policy and if you have any questions contact the Insurance Company requesting a response from them in writing. There are a number of different policies written and you need to know what your coverage is (example: annual deductible, your financial out of pocket expenses, lifetime maximum, etc) Indemnity, Preferred Provider Organization, Point Of Service, Health Management Organization. In some plans you can have in and out of network benefits, known as a Swing Plan. We have a contract with BlueCross/BlueShield, Health Management Network, and currently do work with many other carriers. Example: If you required this procedure and we are not listed as a contracted provider, in many cases we are able to do a "Letter of Agreement", or arrange a "Discount Rate" with your carrier. In most policies if they cannot provide necessary medical care within the network, the carrier will grant the policyholder a waiver to go outside the network for their treatment. The medical claim is then submitted and processed as if the policyholder stayed inside the network. We also work with Industrial Carriers, as we feel the sooner the injured worker is able to return to work the happier everyone involved will be, especially our injured worker. Please note that each state handles their industrial workers' claims according to its rules and regulations, but in most cases Arizona may have a reciprocal agreement. Each State has its own laws and regulations governing all types of insurance. You need to check your State Insurance Department if you have any concerns regarding your carrier. The Self-Insured Employer's responsibilities are different than your general group policy with a small business. Your State Insurance Department enforces the laws, as well as providing the public with information about insurance. We work very hard to assist the patient in obtaining the necessary authorization(s) required for treatment at our facility. If you are unable to arrange to see Dr. Yeung in person, but would like for Dr. Yeung to review your MRI and medical records there is a fee of $250. Dr. Yeung evaluates every case individually as not all patients require surgery. |
