Another aspect of the SED technique that Dr. Yeung uses to determine surgical indication is using evocative diagnostic injections and surgical indication. A lot of cases in Workman’s Comp will try to challenge discography as a legitimate procedure, one that he disagrees with.
This video shows the value of evocative discography in selecting patients needing endoscopic decompression. It illustrates how Transforaminal Epidural Steroid Injection (TFESI) and discography provided evidence of a surgically treatable condition that was made possible and indicated by discography. Good relief with a TFESI and positive evocative discography at the painful disc segment will help predict a positive and favorable response to SED. Chromo-discography is extremely valuable for endoscopic foraminal decompression of the lumbar disc.
This patient had a far lateral herniated disc at the L2-3 that was suspected on the MRI, but his symptoms were difficult to locate dermatome. He responded to TFESI and elected to have SED due to the positive response. Over 1 year later, the patient left a review on Facebook, reporting on his ‘amazing’ recovery.
January 14, 2014 – A year ago last month I was out of work for 2 months and couldn’t walk without a cane, after 2 hours on the table at DISC w Dr. Yeung performing a Selective Endoscopic Discectomy which was totally painless during and after, he woke me and said “get up”, I did and basically walked out of the place. In a week I could walk around the block, in a month I could walk a mile, in 2 months I could jog and do free weights.
